We are seeking a Social Worker / LCSW to provide emotional support to parents and caregivers who provide care to children with serious illnesses and to connect them with social service agencies in their community to meet their needs. The Social Worker / LCSW will provide direct casework, counseling services and psychosocial assessments for patients and families. The Social Worker / LCSW will also play an integral role in participating in interdisciplinary team meetings once per week at our Barrington home office.
No on-call or weekends! This is a Monday thru Friday, day-shift position in conducting patient home visits in the Aurora, Bolingbrook and Lombard, Illinois area. - Master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Licensed as a Clinical Social Worker / LCSW in Illinois
- Minimum of two (2) years clinical experience preferably in a pediatric setting
- Home and palliative care experience preferred
- Reliable vehicle, current driver’s license and liability auto insurance
