Position Description:

Positions in this function include LPN/LVN roles that identify, coordinate, or provide appropriate levels of care under the direct supervision of an RN or MD. Function is responsible for clinical operations and medical management activities across and continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management coordination of care, and medical management consulting.


Position Responsibilities:

• Function may also be responsible for providing health education, coaching and treatment decision support for members.


• Basic knowledge of theories, practices and procedures in a function or skill


• Performs to routine or standard requests


• Uses existing procedures and facts to solve routine problems or conduct routine analyses


• Depends on others for instruction, guidance or direction


• Basic structured, standard approach to work.

Skills / Requirements

Requirements:

• HS/GED


• Must have a valid unrestricted LPN License


• Microsoft Excel, and Word


• Previous Data Entry Experience


• Previous Experience in working in a clinical atmosphere

Assets:

• Bachelors Degree preferred


• Previous experience in case management


• Chronic Kidney Disease/Dialysis case management or clinic experience preferred
